Fabio first appears in Edith, where he begins contacting several of Van’s former 4SPG clients. Believing that Van handled their cases too leniently, Fabio seeks to punish them himself. During this time, he kidnaps Jeffrey Valentino, the former leader of the Desire gang, convinced that Valentino had not been properly punished for the gang’s past crimes. Van and his allies eventually track Fabio and his captive to a junction area in the Lèvre Underground Subway used by the Metropolitan Expressway Authority. After confronting Van and criticizing his current methods, Fabio attempts to recruit him as a partner. When Van refuses, Fabio escapes on his orbal bike, fleeing along Beltway 3.
Fabio later resurfaces in Anchorville, where he begins spreading false rumors about Van to clients connected to his 4SPG work. At the request of ENT Specialist Grant, Fabio kidnaps Dr. Heinemann, the medical director of Anchorville General Hospital. Grant sought revenge against the director for overworking him and forcing him to conceal the existence of a production laboratory constructed by Almata under the hospitals previous administration beneath the hospital. Van and his allies eventually track the pair to the Anchorville Sewer System.
During the confrontation, its revealed Fabio is "contaminated" by Alter Dawn and had been working with Giacomo Conte, one of the Vestiges. At Conte’s request, Fabio spread the false rumors about Van’s activities in Anchorville and had even carried out his earlier actions in Edith under Conte’s direction. After being defeated, Fabio breaks free from the influence of Alter Dawn. He explains that his communication with Conte had been one-sided, leaving him unaware of the Vestige’s true intentions.
Following the incident, Fabio Rosso is taken into custody by the Anchorville Police Department.
